- It was opening day at the Ohio State Fair in Columbus, Ohio
- According to WKBW the accident occurred at 7:19 pm Wednesday
- The "Fireball" ride malfunctioned throwing a carriage of passengers flying into the air.
- Ohio Gov. John Kasich ordered all rides at the fair shut down until each one is inspected.
- A ride known has the G Force (similar to the Fireball ride at Ohio Fair) at the Orange County fair was shut down and reinspected as a precaution, officials said. There was no indication of a problem with the O.C. County ride G Force, but after the Ohio incident, OC fair officials shut the ride down.
